I am participating in the Health Check of Fitsurance B.V. (hereinafter “Fitsurance”), which is conducted on behalf of Fitsurance under the responsibility of Fitsurance personnel. I have been informed of the content and scope of this Health Check.

The check may include the following variables:

• Cholesterol levels, hs-CRP and HbA1c.
• Height/weight, blood pressure and abdominal circumference
• Squint strength and body composition 
• Psychosocial and physical activity questionnaire
• Medical history and lifestyle data

These measurements are (92-99%) accurate. The measurements in no way constitute medical advice, but are the basis for preventive advice regarding your health and lifestyle. When a measurement indicates a possible abnormal value, we refer you to your (family) physician or health care provider. The measurements are taken to provide you with feedback on your general fitness before, during and after the said project.

I give Fitsurance explicit permission to store and process my data within the framework of this health check to receive the data collected with the sent ‘health kit’ directly from the relevant lab and to process these as well. I will also have my own data made available to me in a report and I can request deletion of my data at any time. Twelve months after the health check, all my personal data will be automatically deleted.

I hereby consent that Fitsurance may retain my data anonymously for 5 years for science and research purposes

Fitsurance complies with the guidelines on processing special personal data as described in the General Data Protection Regulation (AVG). This means:

• Voluntary participation, I commit to nothing and withdrawal is always possible
• Implementation and protocols of the vitality check have been clarified to me
• Confidential treatment of my data, without sharing it with third parties
• Privacy policy of Fitsurance can be found at https://www.fitsurance.nl/privacybeleid/. 
• I can indicate at any time that Fitsurance must delete my data
• I can see the measured components of the vitality check myself
• If I have any questions, I can always email info@fitsurance.nl and receive a response within 24 hours on weekdays.

I hereby consent to the processing of my personal data by Fitsurance as described above and I agree to participate in the health check and sign this statement to that effect.
